This is a notice for Social, Health and specific services in accordance with Directive 2014/24/EU Article 74.
North East Hampshire and Farnham (NEHF) CCG and Surrey Heath (SH) CCG, with Associate Commissioners (East Berkshire CCG and North Hampshire CCG), “The Commissioners” are seeking EOI from providers interested in delivering a range of primarily adult integrated community based healthcare services.
The commissioned services are organised into three lots covering differing CCG registered populations.
• Lot 1: This primarily covers the localities of North East Hampshire and Farnham and Surrey Heath CCGs, the services provided for the Associate Commissioner organisations relate primarily to specific requirements within the community bed service – further details are available within the specifications.
• Lot 2: Applicable to the North East Hampshire and Farnham CCG population only.
• Lot 3: Applicable to the Surrey Heath CCG population only.
The Commissioners have a clear strategy for the development of integrated service models for health and social care within each of its localities, and the community provider/s must be an integral part of this. These services will work together to support individuals, their families and carers by providing a fully co-ordinated community based service.
The providers must be able to work towards the continual improvement of services in a way that integrates:
Health and social care
Physical and mental health
Statutory and voluntary services
Provision and commissioning activities
Personal and professional responsibilities
The Commissioners wish to commission only from providers that are willing and committed to this level of partnership working.
Lot 1
This is a joint lot for both NEHF & SH CCGs and comprises;
• Community beds – Farnham & Fleet
• Diagnostic and Treatment Centre
• Out of hours nursing and support services
• Hospital interface team
• Specialist Practitioners
• Respiratory services
Lot 2
This is for the services below, for NEHF residents;
• Community Nursing service
• Therapies & Rehabilitation services
Lot 3
This is for the services below, for SH residents;
• Community Nursing service
• Therapies & Rehabilitation services
Contract Term
Three contracts will be awarded with one for each Lot.
The start dates of the contracts will be 01 April 2020. It is expected that the contract will be awarded for a total of five (5) years with the possibility to extend for up to a further two (2) years.

Selection questionnaire (SQ) responses must be submitted by 09:00am on the closing date Tuesday 2nd January 2019.
