The aim of the Home from Hospital service is to provide short term support, of up to 3 visits and follow up telephone check calls if required, for a maximum period of 2 weeks.
This would comprise of social and practical help to those vulnerable people who feel they may need additional assistance following discharge from hospital – in particular those who may have limited support in their local community.
The service is not intended to provide long term or on-going support or to replace any statutory service to which the individual may be entitled. Nor is it intended to offer help with any personal tasks or to administer medication or change dressings. If the need for such services is identified, the co-ordinator will, with the individual’s agreement, make an appropriate referral.
